Moving to a congregate setting is often an overwhelming
and
emotional transition for seniors. In many cases, they are
leaving behind cherished possessions or homes where they raised
their families and created memories. We offer assistance to make
this change easier and will be there to assist family members in
coordinating every step of the move.
During initial health and function assessments prior to the
resident's move to one of our facilities, medical and dietary
information is gathered as well as personal preferences. Special
care can then be taken to ensure it is as comfortable and smooth
as possible by mimicking the lifestyle patterns and routines
clients are familiar with.
Staff make every effort to link the individual's care plan
with lifestyle preferences associated with appearance, routines,
music, hobbies, or spirituality.
